Personalized Dementia Care Plans

alzheimer society is a national institute

Your family is one of a kind, which means you need a personal care plan designed specifically with you and other family members in mind. In our initial consultation we will sit down with you and go through our documented intake process which involves collecting information not only about what your family needs but about who you are - because every family is different.

We then design a care plan that includes both the Activities of Daily Living (ADL’s) and Instrumental Activities of Daily Living (IADL’s).

We will help you understand the 3 Stages of Alzheimer’s Disease and What to Expect at Each Stage.

Our client, John was diagnosed with Alzheimer’s disease. He was an early riser and sometimes he would try to wander outside before the rest of his family members were up. Because we had spent time with his Family Caregiver up front discussing his childhood - we knew that he was likely thinking about his childhood chores and was on his way to milk the cows. Our intake process gave us the information we needed to assure him that his brother Charlie had all the outside work taken care of and that he could enjoy his breakfast.

Coaching

I could not have gotten through this without my Choice Care Coach. I had her number and could call her whenever I had questions or just to vent. I learned so much about dementia and how to just BE with my wife. I didn’t feel alone or lost. I knew I could call and ask any question when I needed to.” ~Al

Our Dementia Care Coaches work directly with individuals and families facing dementia by providing emotional support, practical advice, and teaching strategies to help with decisions and to access care that’s right for each individual facing dementia.

Your Dementia Care Coach will support you as you face the challenges of today and tomorrow with improved confidence. Dementia is experienced differently by each person and our work seeks to recognize and respond individually to your needs. 100% of our clients report enjoying multiple benefits as a result of their connection with a Choice care coach. The top benefits reported are:

  • Feeling more knowledgeable about dementia.

  • Know more about services that can help their family member and friends stay well.

  • Feel supported as a caregiver.

  • Feel more confident in their caregiving abilities.

In-Home Care

  • In Home Care

In home care service for your loved one with dementia or Alzheimer's. We provide everything from once weekly care so you can get out to run errands or go shopping to full time regular care so that you are confident your loved one's needs are taken care of. Free at home assessment available.

  • Day Time Care

Do you need a break from caring for your loved one with dementia or Alzheimer's? Our daytime care may be the solution - we can come to your home on a schedule that suits your family (once weekly, four hour minimum) so that you can go to work, run errands, spend time with your children or even just have a nap.

  • Night Time Care

The number one concern we hear from Family Caregivers is lack of sleep which causes all sorts of troubling behaviour such as health problems, irritability and the inability to step back and see the bigger picture of care.

Our caregivers can spend the night in your home to address your loved one’s needs so that you can sleep soundly.

  • Medication Assistance

Remembering to take your medications at the right time and the right dosage can be hard for anyone. As we age, and have more complicated medical care needs, it becomes even easier to forget. Missing medications can lead to negative drug interactions, side effects, illness, or even hospitalization. If you find yourself forgetting to take your medication or your medication routine is becoming complicated, we can help.

  • Meal Preparation

Good, healthy, nutritious meals prepared together (where possible), served on a schedule that makes your loved one feel in control and combined with light clean up assistance will remind your loved one of the simple pleasure of eating well. Choice Caregivers can even coordinate trips to the grocery store which serve as both mental stimulation and physical activity for those with dementia.

  • Light Housekeeping

Everyone loves a clean home but not everyone loves to clean. But comfort and safety at home are an important facet of life especially for seniors who wish to age at home. As your loved one’s physical health declines, they may find it difficult to perform normal everyday tasks. Many seniors turn to family members for assistance with daily activities. However, you may not have the time to fit these chores into your daily routine and in fact would prefer to spend quality time with your loved ones. Choice Caregivers can provide assistance with light housekeeping or we can coordinate ongoing cleaning to relieve you and your loved ones from this burden.

  • Bath Assist with Dignity

We all know how good a hot shower or bath feels, but for some seniors, bathing may become more challenging or even something disagreeable. Our team is trained to assist in all forms of personal care including bathing, dressing, toileting and continence while maintaining your loved one’s personal dignity.

  • Engaged Memory Care

Engaged memory care puts the needs of the person living with dementia first and includes: *listening to their needs, concerns, and desires *offering a sense of routine and purpose *focusing on self-esteem and a sense of accomplishment

We follow the principle of "nothing about us, without us".

  • Companionship & Theraputic Recreation

Our goal is to ensure your loved one not only survives but thrives. Therapeutic recreation is a key to all of our mental and spiritual well being.

So if your father loves chess, we will play - we know it helps develop problem solving skills. If mom likes to garden, then let’s dig in the dirt. And if you are looking for ways to help your loved ones maintain their connection with your children and their grandchildren, arts and crafts can be an excellent tool.

Our team has been specifically trained to speak slowly and in a reassuring tone as well as to read your loved one’s body language so that we can anticipate behavior changes.

Long Term Care Homes

creating a family caregiver alliance can be beneficial

“After my mother moved into a long-term care facility, we reached out to Choice. Cyn worked with us and the care home staff to create a program for my mum that augments care home services . Without that I do not think the transition would have gone well for my mum ” ~Rebecca

We are often asked by primary care givers to continue our work with their loved-ones, once they are placed in a care home, which has proven to reduce depression, stress and anxiety; recover basic motor functioning; build confidence; and create opportunities to socialize.
